Looking Up Program

Providing support for chronic disease

self-management

The Looking Up program, a chronic disease self-management education program, made significant strides in 2022, enrolling 11 participants in the spring cohort and an additional 10 participants in the fall cohort. A key factor in our success was the continued collaboration with the North Carolina Cooperative Extension service, which provided essential tools and education, with a focus on nutrition and physical activity. In tandem, our dedicated nursing students and faculty conducted specialized sessions addressing diabetes and hypertension, ensuring a comprehensive approach to our clients' health.

The outcomes of the program were encouraging. Many participants achieved their health-related goals, experiencing positive transformations such as weight loss, improved cholesterol levels, lower HgA1c levels, and reduced blood pressure. One individual, in particular, stands out for reducing his lifetime heart disease risk score dramatically – from 36% to an impressive 5% risk. This remarkable progress highlights the program's efficacy and the dedication of both participants and program facilitators.

The support and active engagement of our nursing students have been invaluable, reflecting their commitment to making a tangible difference in the lives of those we serve. As we move forward, we remain steadfast in our mission to empower individuals to take control of their health and well-being, and we're grateful for the collaborative efforts that have made the Looking Up program a continued success.
